Abstract

Competency-based medical education seeks to ensure that graduates reach practice prepared to care for their patients safely. To synthesize the evidence on the competencies of medical graduates, a systematic review of articles published between 2021 and 2026 in SciELO and Scopus was conducted according to the PRISMA stages. Thirteen studies from Peru, Brazil, Mexico, the United States and Taiwan, including two international reviews, formed the corpus. Three axes emerged: perceived preparedness and competency gaps, assessment through entrustable professional activities, and curriculum and accreditation. Graduates feel prepared for general clinical practice but show gaps in emergencies, management, patient safety and communication, and tend to overestimate their performance. Workplace-based assessment and curricula aligned with health needs are required.
